PATRICK AND ANNA M. CUDAHY FUND
APPLICATION GUIDELINES
The Patrick and Anna Cudahy Fund is a general purpose foundation which primarily supports organizations in Wisconsin and the metropolitan Chicago area . The principle areas of interest are social service, youth, and education with some giving for the arts, and other areas. Some support is also given for local and national programs concerned with public interest and environmental issues. A few grants are given for international programs but only to those which are represented by a United States based organization..
REQUESTS ARE NOT CONSIDERED FOR:
- Organizations and projects primarily serving a local constituency outside of Wisconsin and the Chicago metropolitan area.
- Organizations outside the United States who are not represented by a United States based 501 [c]3 organization.
- Grants to individuals.
- Loans or endowments.
PROPOSALS MUST INCLUDE:
- Summary of Request: Completed one page Cudahy Fund form entitled `Summary of Request for Funding'. Summary must appear on this form.
- Proposal Narrative: A brief description of the organization and an outline of the proposed project, which may not exceed five (5) pages in length.
- Budget: Separate, projected income and expense breakdown for the organization and the project.
- Major Funding Sources: A list of major institutional donors to the organization during the past fiscal year. Maximum of ten(10) names with amounts.
- Board of Directors: A complete, current list of board of directors of the organization.
- IRS Classification: A copy of the Internal Revenue Service ruling that the organization has 501 [c]3 status.
- Financial Statement: A complete copy of the most recent audited financial statement. If the organization is not required to be audited: a detailed balance sheet of actual income and expenses, assets and liabilities for the past fiscal year.

SUBMISSION OF PROPOSALS:
- Deadline Dates: January 5 (March Bd.); April 5 (June Bd.); July 5 (September Bd.); October 5 (December Bd.).
- Proposals must be POSTMARKED by the above dates.
- Organizations may submit only one proposal during any calendar year.
- No proposal will be considered unless all the required materials have been received in our office. Do not send video tapes, cassette tapes, or multiple copies of the proposal.
- We do not accept proposals via fax.
